If your child needs to be excused from Physical Education, a note is required.
 

• A parent/guardian note is sufficient for up to one week.
• In order for your child to be excused for longer than one week, a doctor's note is required. The note must state the reason why, and length of time your child will need to be excused. "Until further notice" is not acceptable. If the doctor prefers to leave the excuse open-ended, he/she should excuse the child until the next scheduled re-evaluation. After that evaluation, a new note will need to be submitted to the Health office.
